PUERTO RICO DEPARTMENT OF EDUCATION (BEN #157779) INVITATION TO QUOTE PRICING FOR E-RATE EQUIPMENT AND SERVICES FY2024 CATEGORY 2 FORM 470 #240022975 QUOTE DUE DATE AND ALLOWABLE CONTRACT DATE (ACD) – MARCH 22, 2024
The Puerto Rico Department of Education (“PRDE" or the “Department") posted E-Rate FY2024 Form 470 240022975 on February 23, 2024 and would like to invite your company to submit a quote if you are interested and meet the qualifications below. The Form 470 can be found on the USAC EPC portal at USAC.org/sl, and a copy is attached to this notice, along with the service/product specifications, and the pricing workbook. The products and services requested are as follows:
Wireless Access Points – Indoor & Outdoor
Access Point Enclosures
UPS Units
Cabinets
Network Switches
Cabling/Fiber
Interested vendors may elect to quote some but not all of the listed items, but must include installation, licenses and all other required components for every category quoted.
The quantities requested in the Form 470 represent the Department's best estimate of the total equipment and materials needed. Actual equipment purchased will be on an as needed basis.
The Department intends to award a contract for three (3) years, with an option to extend for up to one (1) additional year to one (1) or more responsive vendors.
A Notice of Award and Acceptance Agreement must be signed prior to the filing of the Department's E-Rate application, and will be subject to a formal contract approved by all required government agencies.
VENDOR REQUIREMENTS: All responding vendors must (a) have an E-Rate Service Provider Identification Number (SPIN), (b) have been a service provider in the E-Rate program for the past five (5) years, and (c) be registered in SAM.gov.
QUOTE REQUIREMENTS: Each quote must include the following:
1. The name, cellular phone, email address and mailing address of the principal contact for the vendor
2. The vendor's E-Rate Service Provider Identification Number (SPIN)
3. A completed pricing spreadsheet for each category of equipment/product being quoted, including installation, using the Pricing Workbook provided
4. List of the Funding Years the vendor has received E-Rate funding during the past five (5) years
5. Description of vendor experience providing network equipment and installation services comparable in size and scope during the past five (5) years
DEADLINES:
1. All questions on the Form 470 must be submitted in writing to E-RATE_CAT2@DE.PR.GOV  no later than 12:00 P.M. (AST) on Friday March 8, 2024.
2. Quotes must be submitted to E-RATE_CAT2@DE.PR.GOV no later than 12:00 P.M. (AST) on Friday, March 22, 2024.
LATE QUOTES WILL NOT BE CONSIDERED FOR A CONTRACT AWARD SPAM AND/OR ROBOTIC RESPONSES WILL NOT BE CONSIDERED VALID BID RESPONSES AND WILL BE DISQUALIFIED FROM CONSIDERATION
