​The Puerto Rico Department of Education (the “Department") posted E-Rate FY2025 Form 470 #250022409 on February 14, 2025, and would like to invite your company to submit a quote if you are interested and meet the qualifications below. The Form 470 can be found on the USAC EPC portal and a copy is provided along with this notice. The products and services requested are as follows: Cat 6 Cabling & Fiber, including installation. 
The quantities requested in the Form 470 represent the Department’s best estimate of the total equipment and materials needed. Actual equipment purchased will be on an as needed basis, and the Department does not guarantee that it will purchase the quantity of cabling/fiber specified in the Form 470. The Department may purchase less, more or none of the listed cabling/fiber, depending on actual school needs.  As a result, each vendor quote must be for a fixed price for cabling/fiber and installation for the entire term of any contract awarded, regardless of the quantity the Department elects to purchase in any given year. The Department intends to award a contract for three (3) years, with an option to extend for up to one (1) additional year to one (1) responsive vendor. A Notice of Award and Acceptance Agreement must be signed prior to the filing of the Department’s E-Rate application and will be subject to a formal contract approved by all required government agencies. VENDOR REQUIREMENTS: All responding vendors must (a) have an E-Rate Service Provider Identification Number (SPIN), (b) have been a service provider in the E-Rate program during the past five (5) years, and (c) have experience performing comparable cabling and fiber installations during the past five (5) years. QUOTE REQUIREMENTS: Each quote must include the following: 1.     The name, cellular phone, email address and mailing address of the principal contact for the vendor that is legally authorized to sign contracts on behalf of the vendor.2.     A completed Vendor Experience Form detailing cabling and fiber installation services comparable in size and scope performed by the vendor during the past five (5) years, including E-Rate funding and contracts on the form provided. If a bidder intends to subcontract any portion of the work, the vendor must include a completed Subcontractor Experience Form for each subcontractor, and identify the percentage of work to be performed by the subcontractor.3.     A completed Price Sheet of the cabling being quoted, including installation, using the form provided.4.     A bid bond equal to ten percent (10%) of the total bid price must be included with vendor quotes. The bid bond should be in favor of the Department of Education and presented by certified check, postal or bank order, or through an insurance company authorized by the Puerto Rico Commissioner of Insurance.DEADLINES:  1.     All questions on the Form 470 must be submitted in writing to E-RATE_CAT2@DE.PR.GOV no later than 12:00 P.M. (AST) on February 28, 2025.2.     Quotes and required forms must be submitted to E-RATE_CAT2@DE.PR.GOV no later than 12:00 P.M. (AST) on March 14, 2025. LATE QUOTES WILL NOT BE CONSIDERED FOR A CONTRACT AWARDSPAM AND/OR ROBOTIC RESPONSES WILL NOT BE CONSIDERED VALID BID RESPONSES AND WILL BE DISQUALIFIED FROM CONSIDERATION Additional information may be posted regarding the Form 470, so please check the E-Rate EPC portal frequently for updates, including responses to vendor questions.
